The Director Housing Program PCL is responsible for overseeing and coordinating services provided at external facilities. This includes but is not limited to H.E.A.R.T. (Housing Elders at Residences Together), supportive housing sites and cluster housing sites.

Conducts supervisory visits to all Harborlight H.E.A.R.T and supportive housing sites.

Provides skilled nursing care as outlines in the nursing care plan.

Monitors the completion of the caregiver’s orientation, conducts quarterly supervision visits, skill checks and annual performance reviews, to ensure the staff remains in compliance.

Maintain professional relationships with clients, families, case managers, and other staff members

Monitors the budget and provides oversight for all new and existing projects, such as improvements to existing locations as well as the addition of new locations

Ensures that all housing costs are billed timely and accurately

Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed. Develops employees through coaching, mentoring, and formal/on the job training and development opportunities

Performs other duties as assigned.

 

QualificationsRequired

Graduate of an approved school of professional nursing. Current licensed Registered Nurse in practicing state.

Two or more years of management experience preferred in clinical home health services.

Must be a licensed driver in the appropriate state and have available transportation with current insurance coverage.

• Strong organization skills required

• Demonstrated computer literacy skills

• Proficient in Microsoft Excel, Word and Outlook skills
