The Population Health Pharmacy Director (“Director”) oversees the effective implementation and evaluation of all pharmacy-related programming, analytics, and care model development in support of the OneCare’s population health management model. S/he provides strategic insight and direction to senior leaders at the ACO and within the OneCare Network statewide to align with healthcare reform goals. S/he ensures pharmacy strategy and programming align payment reform and health care delivery transformation goals. The Director serves as the senior leader for clinical pharmacy programs such as those related to health outcomes, disease state management, care management, and other specialty programs and the role of the ACO in pharmacy management for all attributed lives. The Director develops and deploys a pharmacy analytics strategy to include advising on value based contract negotiation.S/he builds trusting collaborative relationships with senior leaders, payers, regulators, and stakeholders; facilitates buy-in to OneCare’s pharmacy clinical and quality strategies; and supports change management within and across organizations, communities, and the state. S/he directs clinical leadership workgroups and strategy initiatives with a variety of different teams across the ACO Network. S/he identifies pharmacy trends, designs population health programming and evaluation, and aligns critical stakeholders to ensure program success. The Director engages subject matter experts across clinical disciplines and with administrative leaders to design clinical models of care and supporting business plans.
Licensed Pharmacist in the State of Vermont with graduate training in pharmacy at the masters or doctorate level required. American Society of Health System Pharmacists (ASHP) accredited pharmacy residency training and advanced degree (MBA, MHA) is highly preferred.
This position requires 10 or more years of varied clinical experience in a healthcare related setting (e.g. ACO, MCO, or health plan; continuum of care or healthcare delivery agency) demonstrating expertise in population health management, clinical process improvement,pharmacy data analysis and reporting, and including five or more years of progressive leadership experience. Demonstrated leadership in regional and national pharmacy population health strategy strongly preferred.
